Adaptive streaming (HLS)
Builds a 360p / 480p / 720p / 1080p ladder behind one master.m3u8 so the app can
pick quality from the viewer's connection. Nothing is re-uploaded, and any video
without a ladder keeps playing its MP4 exactly as before.
